Reading the letters

R, Responsible. Does the work. A row can have more than one.

A, Accountable. Owns the outcome and signs off. Exactly one per row; two As is how things fall between chairs.

C, Consulted. Gives input before, two-way.

I, Informed. Told after, one-way. If everyone is C, meetings multiply; most people should be I.

Use role titles, not names, in the header fields. People change; the matrix should survive them. Include decisions and approvals as rows, not just activities.
